Day 19: The Patient Centered Medical Home

There’s no doubt that the current public health crisis is changing the landscape of education. With the curve flattening but a possible smaller-scale resurgence on the horizon, parents should prepare themselves for a new normal. I believe that this new normal will call for the integration of our children’s physicians, like never before. Pediatricians and other primary care practitioners will likely be at the forefront of recommending medically necessary special education services during periods of school closures and incremental re-openings.

According to the American Academy of Pediatrics, Patient-Centered Medical Homes are medical practices that provide comprehensive, advanced primary care, including but not limited to —

  • Developmental assessments, screenings, and parent counseling on nutrition, safety, and psychological issues,

  • Coordination of care between the family, physician, subspecialists, and other services providers to implement a shared care management plan,

  • Interaction with child care and early childhood education programs, early intervention programs, and schools to ensure that the special needs of the child and family are addressed, and

  • Developmentally appropriate and culturally competent health assessments and counseling to promote a successful transition to adult-oriented health care, work, and independence.
